The Artery of Percheron Infarction Presenting as an Vertical Gaze Palsy

Abstract
We report a 37-year-old man who presented with a sudden bilateral conjugate vertical gaze palsy. The horizontal eye movement and convergence were unremarkable. Brain MRI revealed acute infarct on the bilateral paramedian midbrain and anterior thalamus. Brain MRA showed a focal stenosis of left proximal segment of the posterior cerebral artery. A patent foramen ovale was detected by transesophageal echocardiography. These findings suggested that emboli might occlude the artery of Percheron and the structure related with supranuclear eye movement was affected.
